| Which children are eligible for this scholarship? |
FloridaChild Scholarship
Any K-12 grade student who qualifies for free or reduced-price school lunches AND
was counted as a full-time student during the previous state fiscal year OR
received a scholarship from an eligible nonprofit organization OR
is eligible to enter kindergarten or first grade.

John M. McKay Scholarship
Any 1-12 grade student who
has a disability and an Individual Education Plan AND
attended a Florida public school in the prior school year* AND
has obtained acceptance at JMK12 Academy AND
has notified his or her home school district in writing 60 days prior to the first scholarship payment.
* Prior school year means the student was enrolled and reported by the school district for funding in October 2002 and February 2003

| How is eligibility determined? |
FloridaChild Scholarship
FloridaChild determines eligibility based on the income that you reported on your previous year's tax return combined with your household size. For example, a family of four qualifies if their income is approximately $33,485 or less per year.

John M. McKay Scholarship
The Department of Education determines eligibility based on the criteria above. Note: Kindergarten students are eligible if they are repeating kindergarten.

| How long do the scholarships last? |
|
Scholarships will be renewed annually through grade 12 as long as the child and school continue to meet the conditions of eligibility.

| How much is the scholarship? |
FloridaChild Scholarship
For private schools, $3,500 or the cost of tuition, whichever is less.

John M. McKay Scholarship
The scholarship is equal to the private school tuition OR the amount the student would have received under the Florida Education Finance Program, whichever is less.

| How is the scholarship paid? |
FloridaChild Scholarship
Checks are made out to the parent and sent to the school in installments. Parents endorse the checks over to James Madison K12 Academy.

John M. McKay Scholarship
Warrants are made out to the student's parents and mailed to the school on September 1, November 1, February 1, and April 15. The parent must restrictively endorse the warrant to James Madison K12 Academy.

| How do I apply for the scholarship? |
FloridaChild Scholarship
You can apply online at www.floridachild.org or by filling out the enclosed application and mailing it in.

John M. McKay Scholarship
You must indicate your intent to your local school district. You can do this online at www.opportunityschools.org (click on the "Responsibilities" button), or you can fill out the enclosed Intent Form and submit it to your local school district office.
